The following forms are REQUIRED to be filled out prior to your first appointment. Please bring these forms completely filled out with you to your initial visit. If you are unable to download and print the forms we can either mail them or fax them to you or you will need to come in AT LEAST 45 minutes prior to your appointment time. Please keep in mind our office hours are 9-5 with 12-1:30 for lunch. It is important for the midwife to have all your information to review with you. This first appointment is scheduled for an hour. If you do not have your forms filled out proir to your appointment time we will need to re-schedule in order for the midwife to give you the time and attention you will need. You also need to be prepared to pay your initial deposit at this visit.
